Well, they've got it all wrong, because the middle name's supposed to be a patronymic (i.e. saying you're your father's daughter - if your dad's name is Dmitri, your patronymic is Dmitriyevna), and the last name is supposed to be feminized (i.e. if your family name is Smirnov, the women are called Smirnova). That version left out the patronymic and didn't feminize the surname.
